Hepatitis C Virtual Symposium: Primary Care-Based Treatment for People Experiencing Homelessness (2023). Resource Type: Archived Webinar. Description: The Symposium featured speakers from primary care and health care for the homeless settings, national experts on hepatitis C, and a panel of people with lived expertise discussing their own treatment experiences. The symposium provided immediately applicable clinical information on conducting pre-treatment assessments, common barriers to treatment, addressing complex clinical scenarios, models of care delivery, and lessons learned from provider and patient experiences. Breakout sessions addressed getting started with hepatitis treatment, strategies for successful treatment completion, treating hepatitis C in people with advanced liver disease, and addressing common insurance and policy barriers. Along with the Symposium, the National Health Care for the Homeless Council and Swope Health will be releasing a Hepatitis C Treatment Guide to support primary care teams. Clinical Data Elements for UDS eCQMs and their Lookback Timeframes: Useful for EHR transition planning; developed in 2023. (2023). Resource Type: Toolkit. Description: Each electronic clinical quality measure (eCQM) is composed of data elements in the EHR or health IT system that are evaluated according to the measure specifications. It is important to identify what data elements need to be transitioned to any new EHR for clinical quality measure continuity and accuracy. This resource identifies clinical data elements in eCQMs that should be considered when transitioning EHRs.
