The Population Health Management (PHM) Task Force, composed of 13 National Training and Technical Assistance Partner (NTTAP) organizations, was developed to build off of previous work and lessons learned in the Special and Vulnerable Populations Diabetes Task Force. The purpose of the PHM Task Force is to engage health centers, Primary Care Associations (PCAs), and Health Center Controlled Networks (HCCNs) to address critical issues to improve the prevention, treatment, and management of chronic conditions among at-risk patient populations in health centers nationally.
