Resource Summary: The annual Community Health Center Chartbook presents data on Federally Qualified Health Centers, including Health Center Program Grantees and Look-Alike organizations. The chartbook includes summary data on health center patients, services, staffing, quality outcomes, and impact on access to care and health equity.
Resource Details: The chartbook also compares health center outcomes to other providers and national data. Unless otherwise noted, chartbook data comes from the 2022 Uniform Data System, collected by the Health Resources and Services Administration. The chartbook contains nearly 75 pages of charts illustrating different aspects of health centers and their patients, from dental services onsite to percentage of patients with chronic conditions, and much more.
