HIT/Data

Resource List

Community Health WorkerHealth Information Technology (HIT), data, and telehealth are transformative tools for community health centers in the modern healthcare landscape. HIT systems, including Electronic Health Records (EHRs), Practice Management Software (PMS), and Clinical Decision Support (CDS) Systems, enable efficient and accurate collection, storage, and retrieval of patient information, leading to better-coordinated care. Leveraging data analytics allows health centers to identify trends, measure outcomes, and pinpoint areas for quality improvement and targeted interventions. Telehealth expands access to care, particularly for patients in remote areas or those facing transportation barriers, by enabling virtual visits and remote monitoring.
