Description: The purpose of this webinar is to demonstrate an example of a patient-centered model program, such as the Diabetes Education Program at Brownsville Community Health Center (BCHC) in South Texas. More than 90% of BCHC patients are Hispanic and Spanish speaking, with most having a limited education and many challenges related to economics and family dynamics. In this 60-minute webcast, the presenter will share information on the development of this program, how it integrates understanding of the specific needs of the population served, the use of motivational interviewing techniques, and the importance of building relationships to promote lifestyle changes. At the center of the program is acknowledging the value of life experiences and belief systems that adult learners bring to an educational session and leveraging these patient experiences to promote active self-management.
