Meeting the Needs of Puerto Rico Health Centers Learning Collaborative in Spanish


Organization: Migrant Clinicians Network (MCN).

Description: MCN will provide a 4 hour intensive in-person Learning Collaborative for at least 10 health centers located in Puerto Rico to address key resources, strategies, and tools to address emerging issues health centers face. The goal of the learning collaborative is to increase an understanding of how to effectively respond to emerging issues and maintain the services that are critical to clinical practice. This LC will be conducted in Spanish and will be targeted to address the unique needs of Puerto Rico. This LC will build upon the in-depth work that MCN has been engaged with in Puerto Rico and will include MCN’s Puerto Rican staff members as faculty. Session topics will be determined through an analysis of needs expressed during previous Puerto Rican LCs. Content will also be informed by identified gaps in care on the island in the wake of recent natural and public health disasters, with an eye towards building increased resiliency to address essential clinical and social issues when future natural disasters strike.

Navigating Neighborhood Context and Transportation Social Drivers of Health Learning Collaborative

Participants will also have the opportunity to review and evaluate a closed-loop SDOH referral protocol that can be implemented at their health centers. Patient Experience Assessment Tool


Organization: National Nurse-Led Care Consortium (NNCC).

Description: With the Advancing Health Center Excellence Framework in mind, this activity will address population health with a focus on the domain of patient experience. NNCC will develop and conduct a Learning Collaborative focused on addressing the patient experience as a key to advancing health equity. A minimum of 10 Health centers and participants will be able to assess their HC's maturity in providing a high-quality patient primary care setting. Participants will explore an HC assessment tool developed by NNCC to align with the AHCE framework's domain of patient experience and subdomain areas. HC will self-assess and then develop a small test of change to create improvements over the 3-6 month period.

Postgraduate Nurse Practitioner (NP) Residency and NP/Physician Assistant (PA) Training Programs

Description: The Postgraduate Nurse Practitioner (NP) Residency and NP/Physician Assistant (PA) Training Programs Learning Collaborative is a 6-month participatory learning experience designed to provide transformational strategies and coaching support to help health centers and look-alikes implement postgraduate NP and NP/PA residency programs. The Collaborative will provide a series of videoconference learning sessions with 10-12 health centers and look-alikes from across the country, as well as quality improvement training, ongoing mentoring for the coach in the organization, technical assistance, and access to web-based tools. The action periods between sessions will include assignments and deliverables, and coaching calls between health center coaches and CHCI mentor coaches to facilitate the uptake of the intervention.

Psychosocial Behavioral Health Peer Support Learning Collaborative


Organization: Migrant Clinicians Network (MCN).

Description: Clinicians, therapists, and social workers are just a few examples of providers exposed to traumatic stories in their daily work. In the course of doing their jobs, these helpers are often in the position of witness. Witnesses experience distress similar to what victim’s experience. Sometimes the distress comes from the stories clients tell them or directly observing their clients in interactions. Sometimes the distress comes from the people who administer the policies and procedures that affect clients. Often the distress derives from both sources. “Helpers” or providers may also have their own challenging histories. Current situations may trigger difficult historical moments, making it harder for providers to cope with contemporary stress. It is critical that health centers develop a workplace environment that provides support, tools, and empathy for providers working in their systems. To begin to address this issue, MCN and expert faculty, will develop a four-part Learning Collaborative (LC) for clinical supervisors and other health center team members at a minimum of 10 health centers. In this LC we dive deep into methods of strengthening peer support in the workplace, the single most important factor associated with well-being under challenging circumstances. We look at the nuts and bolts of bolstering “reasonable hope” and amplifying existing resilience styles and discuss strategies and frameworks to build connections and support among staff. Faculty members will engage participants in brainstorming solutions together. The LC will be highly interactive with a well-developed curriculum, homework between sessions, and a series of resources for additional information and skill building.

ACU will serve as subject matter experts as a part of a learning collaborative focusing on the recruitment and retention of dental assistants hosted by the National Network for Oral Health Access (NNOHA).

School-Based Health Center Start-Up

Description: The School-Based Health Alliance is excited to announce a new learning collaborative focused on school-based health center (SBHC) start-up. This initiative aims to support partnerships between healthcare organizations and schools to provide or expand access to primary care and behavioral health services in schools. Throughout the learning collaborative, participating teams will: Learn about the core competencies for SBHCs and practices to support SBHC sustainability. Identify areas of strength and areas for growth. Identify action steps for strengthening partnerships, improving sustainability, and maintaining high-quality care. This learning collaborative is free and open to school-based health center planning teams, with a focus on Health Center Program and Look-Alike health sponsors. Other health sponsor types are welcome to apply as well. “Health sponsor” refers to the healthcare organization whose staff will provide health services (primary care, behavioral health, etc.) in the SBHC. Teams with existing SBHCs who feel they would benefit from revisiting SBHC basics are also welcome to participate. The learning collaborative will meet on select Tuesdays between January and March 2024. Continuing education credits will be offered. Applications are due November 16, 2023.

Successful Steps for Holistic Integration of Mental and Behavioral Health into Primary Care

Description: With the Advancing Health Center Excellence Framework in mind, this activity will improve quality patient care and safety, build health center workforce competence, and address population health through continuity of care and comprehensive and timely services. NNCC will develop and conduct a Learning Collaborative in partnership with the Primary Care Development Corporation on how health centers can successfully integrate mental and behavioral health into primary care to improve access to quality care. We will explore promising practices and strategies that can be adapted into practice guided by our SMEs and faculty—adapting operational integration of behavioral health into the primary care of those who access community health centers, including the unhoused and residents of public housing, while increasing service and ROI. During the LC, participants will have the opportunity to create a small PDSA activity to enhance services to support mental and behavioral health.

Teledentistry for Health Equity Learning Collaborative

Description: The learning collaborative will focus on best practices to improve existing teledentistry models through more efficient practices, patient engagement, provider recruitment/retention, and clinical improvements. NNOHA will partner with PCAs that have legislation that allows for teledentistry in order to recruit health centers and garner active participation. This learning collaborative will also discuss how teledentistry can be used to improve health equity by creating a care model that provide equitable access to all populations. The Role of Identifying the Social Drivers of Health In Advancing Health Equity
