Resource Summary: When youth with disabilities age out of pediatric care provision, they often experience healthcare disruptions. Knowledge gaps for both families and providers persist, but increased healthcare transition activities can make a difference in healthcare and quality of life outcomes for youth with disabilities. It is important to understand healthcare transition frameworks and implementation strategies.

Participants will define Got Transition’s elements of Youth Healthcare Transition. 2. Participants will analyze how their practice or clinic could apply elements of YHT. 3. Participants will practice introducing youth healthcare transition to youth and their families through a case vignette.
