Resource Summary: With the focus on improving diabetes outcomes with team-based care, the Renaye James Healthcare Advisors’ training focuses on increasing the use of population health platforms to identify patients, communicate outcomes and results, increase patient engagement, and measure performance. A population health platform can also be used to measure and evaluate diabetes care management processes and outcomes. Understanding how to use population health tools to identify diabetic patients through risk stratification and those due for preventative appointments is also addressed.

Resource Details: This training will teach health care providers how to use a digital population health application to identify and stratify diabetic patients, identify diabetic patients who are due for preventive appointments and those with care gaps, and how to use health platform to communicate results and patient engagement regarding care.
