Resource Summary: This case study presents the PHM implementation story of Baldwin Family Health Care (BFHC), a network of five primary care sites in Michigan. BFHC implemented the i2i Tracks Population Health Management Platform, which connects to EHRs to extract and aggregate clinical and financial data, identify at-risk populations, assign personalized actionable care pathways, and engage patients in team-based care. BFHC’s goal was to improve the health of their patient population. As a result of their population health implementation project, BFHC earned the earned the prestigious Level 3 NCQA-PCQH accreditation within just 24 months.
