Drawing from our publication, “Tools for Creating an Environment that Supports Integration,” released in 2020 and the recently released, “Optimizing People, Place and Process: Resources to Facilitate Integrated Care in FQHCs,” we’ll present guiding principles for creating a place in which integrated care teams can work effectively with each other and the patients they serve.
