Description: This hub is designed to provide information and resources for farmworkers and their families on some of the most common infectious diseases and their associated vaccines such as COVID-19, influenza, tetanus, diphtheria, pertussis, and tuberculosis.  The resources included in this hub provide information on the symptoms, transmission, prevention, and the importance of vaccination available in English, Spanish, and low-frequency and Indigenous languages such as Mam, Haitian Creole, K’iche, Purépecha, Mixteco, Triqui, among others.  Materials have gone through a process of assessment to ensure they are culturally and linguistically appropriate for agricultural workers.  More Details...

Housing and the Medical Respite Care Program: A Practical Guide to Navigating the Homelessness Response System (2023). Resource Type: Publication. Description: Medical respite care (MRC) is an essential part of the health care and homelessness response system’s continuum for unhoused community members. Medical respite care is defined as acute and post-acute care for people experiencing homelessness who are too ill or frail to recover from a physical illness or injury on the streets, but who are not ill enough to be in a hospital. While programs vary in size and structure, they are all guided by the Standards for Medical Respite Care Programs (the Standards) and share the same fundamental element: short-term residential care that allows people experiencing homelessness the opportunity to rest, recover, and heal in a safe environment while accessing medical care and other supportive services. Within the Standards, Standard 5 states, “Medical respite program assists in health care coordination, provides wraparound services, and facilitates access to comprehensive support services.” Additionally, Standard 6 states, “Medical respite program facilitates safe and appropriate care transitions out of medical respite care.” Unfortunately, there are challenges in meeting and exceeding these two standards due to a significant lack of affordable housing that meets the needs of various community members within the United States (US), and the complexity of the homelessness response system and housing process to access available housing. Interoperability Readiness Scorecard: HITEQ Center, July 2023 (2023). Resource Type: Template. Description: Many health centers struggle to reap the benefits of technological advancement and investments in health information technology (health IT), while others embrace them and reap rewards. Interoperability is one such example; requiring health centers assess systems, relationships, and implementation. There are keys to successful interoperability implementation for which health centers must develop processes, stand up infrastructure (within the system, internally and externally, and organization), and then take action. Process refers to structured processes, policies, and procedures within the health center. Infrastructure refers to structural capacity and ability within the health center’s technology and staffing structure. Action refers to full implementation to the point of active and ongoing use and engagement. This scorecard encourages health centers to consider their processes, infrastructure, and action in a number of key areas. Each area key to interoperability are to be self-graded on a scale of 1 through 5, where 1 is poorly or not yet developed and 5 is well developed. Strategies for Emergency Management of Chronic Conditions Among Agricultural Workers (2023). Resource Type: Publication. Description: People living with chronic conditions, and especially Migratory and Seasonal Agricultural Workers (MSAWs), can encounter particular threats to their health during an emergency event. NCFH has developed a resource to assist health center staff, clinical coordinators, and emergency managers with tools to assist MSAW chronic care patients, so that the worst outcomes of these events can be avoided. This checklist includes risk factors for chronic care patients during an emergency, action items to help patients prepare, and disease-specific recommendations for diabetes, cardiovascular disease, and chronic kidney disease.
