Resource Summary: The Patient-Centered Medical Home (PCMH) model of care is a way to organize healthcare delivery, coordinate care for patients, ensure the patient's needs are identified, and optimize healthcare outcomes. Despite the various certifications, many feel that a true PCMH is not attainable. This learning collaborative provides a first step in assisting health centers with organizing their approach to communication of the model with their healthcare teams and incorporating the standards into everyday operations. This Learning Collaborative aligns with the Health Center Performance Improvement domain of Quality, Patient Care, and Safety as it reviews how to improve operations to positively impact healthcare outcomes. Each session is 90 minutes.

Resource Details: This session provides tools recommended by Renaye James Healthcare Advisors to operationalize PCMH. The tools will include, but are not limited to, staff roles and responsibility assignment tools; a RACI chart/matrix; a huddle tracker; a staff allocation spreadsheet; a time-out form; and a PDSA tracker. Each session will provide the participants an opportunity to identify the first steps in their organization to develop standard procedures or workflows for PCMH.
