Resource Summary: This is the final webinar of a three part learning series for health center clinicians and care team members serving Pacific Islander patients with diabetes. The following learning collaborative intends to inform and disseminate successful interventions for Pacific Islander patients impacted by uncontrolled diabetes (HbA1c>9) as prioritized by AAPCHO and the Health Resources Services Administration Bureau of Primary Care.
Resource Details: After watching this webinar, participants will be able to 1) Increase understanding of the unique challenges in coverage that impact Pacific Islander patients with diabetes.; 2) Identify culturally appropriate approaches to increase medication adherence for Pacific Islander patients with diabetes.; and 3) Increase knowledge of kidney-related complications associated with diabetes for Pacific Islander patients and approaches health care teams can implement to support management as well as opportunities for prevention.
