Resource Summary: This webinar is the kick off to the Special and Vulnerable Populations - Diabetes Task Force's 2020 National Learning Series. This session is focused on team-based care and is part one of a four-part webinar series on effective strategies to prevent, treat, and manage diabetes among special and vulnerable health center program patients.

Resource Details: After watching this webinar, participants will be able to 1) understand what team-based care is and why it is important; 2) understand how to develop a high-functioning diabetes care team; and 3) learn promising practices in team-based diabetes care.
