Resource Summary: This publication is part of a series of case studies that highlight innovative models for providing enabling services to help reduce barriers to care and address social determinants of health. This case study features the North Carolina Farmworker Health Program (NCFHP) efforts to deploy an Outreach Team to provide a myriad of core enabling services to farmworkers in the field, at home, and in migrant camps. The case study highlights innovative models of providing outreach and care in addition to discussing key funding, policy, and market drivers.
