This case study features Wellspace Health's (WSH) "Health, Access, Referrals, and Transitions" (HART) program. WSH's HART program utilizes case management services to connect patients to care and to make care transitions to critical clinical and behavioral health services. The HART program provides intensive case management and addresses substance use disorders, suicide prevention, provides street outreach, and respite and recuperative services.
