Resource Summary: How do you know if your health center is ready to participate in a clinically integrated network? Health centers first need to be to assess their strategy, goals, staff, tools, and operational capacity needed for success in a value-based care environment. Next, the health center must identify an approach to value based care that clearly identifies who and how the work will be accomplished along with appropriate business tools to operationalize and measure impact and patient outcomes of those efforts.
