In addition, case management may benefit the health care delivery system, housing programs, and reimbursement sources because of its collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy. This In Focus provides an overview of case management; its positive outcomes; the role these services play in enhancing health and housing interventions; the importance of care coordination as an aspect of case management; as well as discussing the implications for practice, policy, and future research.
