Resource Details: There has been increasing recognition that social determinants of health affect throughout the life-course. Adolescents in particular are a vulnerable population, with health behaviors, access, and outcomes shaped by social and environmental contexts. School-based health centers are ideal settings for incorporation of social determinants screening and connection to resources. The webinar reviews the literature for triaging social needs, connecting patients to resources, and utilizing multidisciplinary team members, and proposes a framework for addressing social determinants in the clinical setting.
