Resource Summary: This case study presents the PHM implementation story of La Maestra Community Health Centers (LMCHC), located in San Diego, California. As part of their dedication to delivering the best care for underserved populations, LMCHC decided to join the Million Hearts® initiative to help clients, the CDC, and CMS prevent one million heart attacks and strokes by 2017. LMCHC needed a solution to better manage patients with and at risk for cardiovascular disease. Using i2i, LMCHC shifted to proactive care management to identify patients at risk and care gaps, resulting in immediate discovery of 453 patients that needed hypertension screening. With i2i, LMCHC achieved a decrease in missed care opportunities from 67% to 59% in just a few months.
